Slow turn on(even with current production tubes to the best of my knowledge) takes care of the tube damage concerns. That said, if a standby is desired, my personal preference is the old 'Fender' way. (which is to say just a switch interrupting the B+ after the rectifier or after the first filter node such as #2 above). There are many more complicated methods that also work but I'm a simple guy.
