I just finished the PCB layout. I'll post it on my web page as a gif, suitable for toner transfer methods - or manual methods, it's pretty simple.  
I took the liberty of including a layout for a flatpack transformer to do the stepdown. The board has the capability of being run from 120VAC with the onboard transformer, and also has pads for connecting 12VAC from an external transformer or a 12-16VDC external DC source, both of which are much smaller boards, as the 120VAC section can simply be left off. The heatsink is onboard.  
If I didn't miss anything, there is also a legend with Mouser part numbers for all the parts, including the onboard transformer.
