Hi Skreddy - thanks for the reply. I figure it was made in the mid-seventies. The codes on two of the pots are... 1377602 and 1377332. I believe 137 is the manufacturer code...(CTS?) and the other #'s are the date code... 7602=2nd week of 1976 and 7332=32nd week of 1973.  
 
It has a stereo, (tip, ring, sleeve), input jack.
