The battery (-) goes to the "cold" lug, the output cap comes out the wiper, and the other side of the output cap goes to the DPDT switch. There are several different wirings for bypass switches. The article "The Technology of Bypasses" at GEO enumerates some of these. The original Dallas Rangemaster did NOT use true bypass. To wire it up like the original, connect the output jack signal lug to the center terminal of a DPDT stomp switch. Connect the input jack to both the input of the effect and one side of the section of the switch that the output jack is connected to. Connect the output capacitor to the other side of that switch section. Connect your status indicator to the same side of the other switch section as the output of the effect, and ground the center terminal of that section. This turns on the light when you switch to the effect output.
