I think that you could adapt the Quad Buffer to do all of this. Use one output to drive the amp. Connect a 560 ohm resistor in series with another output driving a small isolation transformer (600:600 ohm)... this would be a balanced line out. A third output could drive a set of headphones. The coils of the 'phones should be wired in series to make a higher impedance load for the IC.
You should use an LM837 or other quad op-amp that has enough drive for 600 ohm loads.
I've been thinking of adding some mods like this to the Quad web page... maybe this is a good time, eh?
