The mystery cap is 47 pF... all it does is roll off the frequency response above 30kHz or so,  
probably to prevent oscillation.  
 
An amp like this one has a lot of gain, especially at high frequencies, and so considerable care is  
needed in the layout and in the grounding arrangments, to avoid oscillation. Do you have an  
oscilloscope? A problem I have met with several times in building high gain amps is that they  
don't oscillate without an input signal, but when  
you have a signal going in, the amp rings at some  
high frequency... without a scope this is hard to  
identify because it doesn't sound like an oscillation, it just sounds 'horrible'.
